How to Report Changes in your Household
One
of the most important Participant responsibilities is to report any changes
in your household within 10 working days. The information below lists
specific information about the types of changes that must be reported
and how to do it. Forms are available for download from this page, or
call 503.802.8333, Option 3, for more information. Be
sure to sign and date all completed forms.
To report a
change in income...
You must report
the change in your household within 10 working days. HAP is required
to verify any changes in income. If you do not include verification
we may not adjust your rent. Verification is usually a letter from the
source of income that shows the change. For example: a letter from your
employer stating that your job has ended.
STEP 1
- Complete form #P-41 "Requesting
a Change in Housing Assistance". Click

to obtain one.
STEP 2
- Complete form #P-1 "Statement
of Eligibility". Click to download
the form, or contact the Section 8 office to obtain
one.
STEP 3 - Your employer must complete form #V-1 "Verification of Employment Status" . Without this
verification we can not process your request. Please note: Check stubs or timesheets are not acceptable.
STEP 4
- Check to make sure all forms are signed and dated. Submit all completed
forms to the Section 8 office. Turn in all forms before the 15th of
the month for your rent to be adjusted for the next month. Packets
submitted after the 15th of the month will be delayed an additional
30 days.
To report someone
has left your Household...
You must report
the change in your household within 10 working days.
STEP 1
- Complete form #P-40 "Requesting
Someone has Left Your Household". Click

to obtain one.
STEP 2
- Complete form #P-1 "Statement
of Eligibility". Click to download
the form, or contact the Section 8 office to obtain
one.
STEP 3
- Check to make sure all forms are signed and dated. Submit all completed
forms to the Section 8 office. Turn in all forms before the 15th of
the month for your rent to be adjusted for the next month. Packets
submitted after the 15th of the month will be delayed an additional
30 days.
To add a child
to your Household...
You must report
the change in your household within 10 working days of the birth, adoption,
or court-awarded custody of a child.
STEP 1
- Complete form #P-35 "Adding
a Child to Your Household". Click to
download the form, or contact the Section 8 office
to obtain one.
STEP 2
- Complete form #V-27 "Certification of Citizen Status"
for the child to be added. Click on the link to download the form, or contact the Section 8 office to obtain one.
STEP 3
- Check to make sure all forms are signed and
dated. Submit all completed forms to the Section 8 office. Turn in
all forms before the 15th of the month for your rent to be adjusted
for the next month. Packets submitted after the 15th of the month
will be delayed an additional 30 days.
To add an adult
to your Household...
Please contact the Section 8 office (503.802.8333, Option 3). After we receive your request, we will see if
the person that you want to add meets eligibility requirements. We will
also contact your landlord to see if they will approve adding this person
to your lease. After your submit your request, HAP will mail you a letter
to set up an appointment for you to meet with a Housing & Community
Specialist to complete the process.
